• Welcome to Health Score

The Preact Health Score tries to break down the complexity of health into your top health factors, your biggest risk factors, and any individual or family conditions that may be affecting your health. The survey is currently intended to take once, but we will update in the future for you to update individual pieces to see score updates.

Navigation is designed for the desktop but can be used on mobile. Not all questions are mandatory so feel free to skip or choose an answer that isn't revealing. Lastly, for multiple selection values, you can search to narrow down your choices. There are a few limitations here but on mobile devices, be sure to check the mobile prompts. You can take the survey without an account, and if you choose to create after, your score can still be saved.

Lastly, we are constantly making tweaks to the scoring with new data. That said, 0-100 is generally the scale (higher score = better), but you may score over if you are very healthy or you forget some risk or comorbidity factors. Any feedback is greatly appreciated.

PS: SurveyCircle users receive points for their participation, which can be used to recruit free survey participants at SurveyCircle.com
